A CONCUSSION is a type of traumatic brain injury caused by a bump, blow or jolt to the head, face, neck or body that causes the head and brain to move rapidly back and forth and can alter the way the brain normally functions.

This online form should be completed and submitted any time a concussion is suspected following an incident which occurred during a judo related activity. In the case of an incident that occurs during a competition, the host medical team and the athlete's coach should jointly complete this form. For incidents that occur during a training session or other type of judo activity the athlete's coach should complete the form.

The information obtained will remain confidential and will be used to track the following:

1. Participants concussion status
2. Ensure an appropriate medical assessment letter is submitted to Judo BC
3. The participant (their parents), their coach and club are all informed of the concussion and informed of the required Return to Play Concussion protocols
4. A clearance letter are submitted to Judo BC prior to the participant return to the mats.
